Registration forms for Spring Lake , Middletown Classes And Freehold  Classes

Please click on the following link to register your child  online for classes. After you click on Submit, you will be taken to a confirmation page where you will be able to send a Check for $ 25 registration fee and first month payment  or Pay by Credit Card to complete the registration process or you may mail the check in to the address below. We will the forward on a written confirmation to you with class times, etc. Please Note on Online Form MDL is Middletown Studio and SPL is Spring Lake Studio

You may print out the above Form and mail it with a check for Registration fee and first month's fee to the address below.  Place in class is reserved upon receipt of the form and payment.

Fees are based on a full seasons of lessons from Sept to June broken up in monthly Payments. You are responsible for the monthly payment as long as your child continues to attend class. You have the option to make up missed classes.
